We would like to express our appreciation for New Harbinger’s willingness to make a bold move in publishing a book for primary care providers. We would also like to thank our clinical and research colleagues at Group Health Cooperative of Seattle, for supporting our initial exploration of primary care behavioral health integration. Coworkers at Yakima Valley Farm Workers Clinics (Patti) and Central Washington Family Medicine (Kirk) have contributed in numerous ways to the creation of this work. Of special mention, of course, is Deb Gould, who has worked tenaciously to help us translate the newest cognitive and behavioral interventions into a language and format for primary care. Dozens of other primary care providers in community health centers throughout the United States and in military medical treatment facilities around the world have also shaped our thinking. Physicians and nurses in the Veteran’s Administration medical system, numerous health maintenance organization (HMO) systems, and providers in individual medical practices have also listened to us and evaluated and refined our thinking. Most recently, we’ve had the privilege of working with many capable primary care providers in San Francisco Department of Public Health clinics. We offer gratitude beyond words to all of these physicians, midlevel providers, nurses, and their primary care behavioral health colleagues.
