Foreword
Menopause is a time of transition. For most women, it begins five to ten years after midlife and lasts an average of four years. It is a time of hormonal change, and it represents the natural transition from a reproductive to a nonreproductive phase of life. Yet, this change does not occur without announcing itself with a range of signs and symptoms. In the first phase, the natural waxing and waning of hormones across the monthly cycle becomes more erratic, and with these changes comes a host of symptoms that prominently include hot flashes, night sweats, sleeping difficulties, mood swings, and challenges to sexual functioning and enjoyment. As the body transitions further toward full menopause, the menstrual cycle may be missed for two or more months, and symptoms may intensify from the early soft notes of change to a fuller orchestra of symptoms proclaiming your transition to postmenopausal life. And while this change of life is as natural as the sun passing across the sky, the timing, intensity, and degree of impairment of the accompanying symptoms may not strike any two women alike. To meet the challenge of the menopausal transition, an individualized approach to coping with symptoms is needed.
This individualized approach is exactly what The Cognitive Behavioral Workbook for Menopause is designed to provide. This workbook makes clear that there are a wide range of approaches to treatment, and while this book steps forward with the strengths of a cognitive behavioral approach to coping, it does so while also discussing the benefits of a range of alternative therapies. Hormone replacement, acupuncture, and natural therapeutics, like red clover, black cohosh, omega-3 fatty acids, and Saint-John’s-wort, all get their due. Likewise, this workbook does not assume that one treatment size fits all. Women will differ in which symptoms are most pronounced during any one phase of the menopausal transition, and likewise, different elements of this workbook, treatments, or combinations of treatment may be most relevant for any given phase. To meet this variability, women need to be educated about menopause, its symptoms, and the stages of the menopausal transition. This workbook offers information and interventions grounded in the latest scientific studies.
The workbook is written by a team of experts, bringing you perspectives from women’s health, clinical psychiatry, and clinical and health psychology. Likewise, their clinical work includes pharmacologic as well as the cognitive behavioral interventions that form the core of this book. The result is readers are assured to get expert information that is informed by the latest scientific findings and is complemented by case examples providing a personal perspective on symptoms and coping.
As for the clinical content of this book, I can’t express how pleased I am that this workbook makes cognitive behavioral interventions so readily available. Cognitive behavioral therapy offers individuals with a wide range of disorders some of the best outcomes in psychiatry. It is a focused, practical, and collaborative approach that takes a woman from her current experience of symptoms and steps forward with a framework for greater coping. Accordingly, for the varied mood and somatic symptoms of the menopausal transition, a cognitive behavioral approach provides a good fit for the changing targets of distress that define this time of vulnerability. As the authors discuss, any one chapter may have greater or less interest, depending on the phase of menopausal transition and the symptoms that are most prominent in a woman’s life at that time. Accordingly, the importance of individual chapters of this book will change over time, as readers match the information provided to their specific needs.
The workbook format ensures that women don’t just learn about treatment abstractly but are engaged in the practice of change that makes a difference. So, whether it is learning a more mindful approach to hot flashes, reducing their likelihood of occurrence, or keeping a fresh shirt handy for an especially drenching episode, women will learn a toolbox of approaches for keeping mood and life goals on track during this vulnerable time.

Acknowledgments
My understanding of the cognitive behavioral model began early on in my training, in graduate classes, workshops, and readings from cognitive experts. However, it was not until I engaged in my first clinical practicum experience, treating patients with anxiety disorders, that I witnessed just how powerful this theory was when put into practice. People’s lives significantly changed as a result of participating in CBT, and being in a position to facilitate this change motivated me to specialize in this area. I feel that it is important to acknowledge the mentors who helped me develop and hone my skills as a cognitive behavioral therapist, including Dr. R. McCabe, Dr. M. Antony, Dr. P. Bieling, Dr. T. Hadjistavropoulos, Dr. N. Rector, and Dr. M. Lau. Thank you for your guidance, challenges, and patience. You have helped me think outside the box in terms of implementing traditional CBT treatment protocols, to hopefully reach many more individuals experiencing distress.

We would like to acknowledge and express our gratitude to the following individuals who shared their menopausal experiences by responding to our survey: Barbara Hammond, Sharon Lloyd, Gail McCabe, Edna Toth, Teresa Vall, Ghislaine Yergeau, and Elaine Green. Also important to acknowledge are the participants in the CBT group for menopausal symptoms, whose experiences and responses to treatment helped shape the content of this book.
We would like to thank the editors and staff at New Harbinger Publications, especially Melissa Kirk, for their guidance and valuable feedback while writing this book. We are also grateful to Rachel Strimas for taking the time to read and provide helpful feedback and comments on our manuscript. Finally, we would like to acknowledge and deeply thank our families for their support and understanding during the writing of this book.
