Foreword
It is estimated that 1.4 million people in the United States will be diagnosed with some form of cancer in 2007. Everyone has a friend or family member who has been diagnosed with cancer. One of the most difficult problems confronting cancer patients is the depression and anxiety that often accompany being diagnosed and living with the illness. We are all aware of how a diagnosis of cancer can affect the health, happiness, and quality of life of the diagnosed person as well as their family members. In some cases, the depression and anxiety that occur can be extremely debilitating, pervading every aspect of the cancer patient’s life—including work, family, friends, hobbies, spirituality, and so forth.
Many cancer patients and family members will address these problems by reading as much as they can, asking their doctors and nurses important questions, trying to obtain information through the Internet, and communicating with other individuals who have been diagnosed with and treated for cancer. Some individuals will also choose to use antidepressant and antianxiety medications to cope with these problems. All these strategies can be somewhat useful in reducing the anxiety and depression often associated with the cancer experience; however, the approach set forth in this workbook will help you overcome cancer-related anxiety and depression more completely and definitively, and with the acquisition of skills that will last a lifetime.
The behavioral activation treatment (BAT) approach to depression and anxiety that is presented in this workbook is a highly promising intervention that may be of great benefit to cancer patients. It can be used in addition to or instead of medications. Published research studies have provided strong initial support for BAT’s effectiveness, not only in reducing depression and anxiety, but also in increasing quality of life. Readers of this workbook will identify new behavioral skills that offer the opportunity to complete the “cancer experience” in a positive way and enjoy survivorship.
Dr. Hopko and Dr. Lejuez have worked diligently to bring you this well designed and effective treatment that will teach you the skills necessary to overcome your negative emotions. As you move through this workbook, remember that there is a manageable solution to the depression and anxiety you feel. Following the recommendations of these established clinical psychologists will move you down the path toward a more fulfilling life. Overcome your depression and anxiety using BAT—as many before you have!
—John L. Bell, MD, FACS Director, University of Tennessee Medical Center Cancer Institute Chief, Division of Surgical Oncology
Acknowledgments
Development of this workbook would not have been possible without the efforts of many people. In particular, we are grateful for the work of behavioral activation researchers who are tirelessly working toward developing improved behavioral treatments for depression, anxiety, and psychological distress associated with various medical problems. Included in this list are Michael Addis, Stacey Daughters, Sona Dimidjian, Keith Dobson, David Dunner, Jackie Gollan, Steven Hollon, Mathew Jakupak, Peter Lewinsohn, Christopher Martell, Patrick Mulick, Amy Naugle, Jeffrey Porter, Adria Trotman, and the late Neil Jacobson.
No program of treatment outcome research would be manageable without the efforts of bright and highly motivated graduate students. Thank you to the graduate students who have been a part of our program of research, including Maria Armento, Marina Bornovalova, John Carvalho, Lindsey Colman, Michelle Duplinsky, Melissa Hunt, Samantha Levine, Christen Mullane, and Sarah Robertson.
We also wish to thank the National Institute of Mental Health, the National Cancer Institute, and the Susan G. Komen Breast Cancer Foundation for their support of our research.

CHAPTER 1
Your Cancer Experience
If you are reading this book, chances are that you may already know a great deal about cancer and how it works. However, there are also a significant number of cancer patients who make the decision to avoid learning about and thinking about their diagnosis. As this book progresses, we will address more specifically the concept of avoidance , that is, avoiding certain behaviors or thoughts, and why the human tendency to avoid can create problems. For the time being, realize that we are going to take a different path toward overcoming your anxiety and depression by approaching certain circumstances and behaviors, a process we will refer to as activation .
It is this mindset that will enable you to move forward in your life and to learn skills that will better allow you to cope with whatever difficult circumstances life throws in your direction. To head down this path of becoming activated, let’s begin with the process of understanding the diagnosis of cancer. We’ll include a time estimate for working through each chapter.
