Resumen
El cuidado de enfermedades crónicas complejas (diabetes, EPOC, VIH/Sida...) demanda cada vez más recursos sanitarios. En el caso del VIH/Sida, los problemas a los que se enfrenta una persona infectada han cambiado bastante en los últimos tiempos. Anteriormente, los principales esfuerzos se empleaban en aumentar el tiempo de vida de los pacientes, mientras que ahora se pretende mejorar su calidad de vida, afectada por múltiples factores (médicos, psicológicos y sociales).
El Hospital VIHrtual es un sistema de telemedicina que mejora el seguimiento y cuidado a domicilio de pacientes con VIH/Sida. Los principales servicios que el sistema ofrece tanto a los pacientes como al personal sanitario mediante Internet son los siguientes: las consultas (por videoconferencia, por chat y por mensajes), la visualización de los datos del paciente, la gestión de citas, la telefarmacia, las comunidades virtuales y la biblioteca. La principal innovación del sistema es que abarca todo el proceso de cuidado del paciente de forma global por medio de Internet (consultas, seguimiento médico, psicológico y social, medicación, calidad de vida, coordinación del equipo de cuidado, etc.) sin pretender grandes innovaciones tecnológicas sino de servicios, ya que se emplean tecnologías probadas y de bajo coste.
Durante dos años se está llevando a cabo una experiencia piloto en el Hospital Clínico de Barcelona con un total de cien pacientes (cincuenta en el grupo experimental y cincuenta en el grupo control, cruzándose al año) y veinte profesionales sanitarios (médicos, enfermeros, psicólogos, psiquiatras, trabajadores sociales, farmacéuticos, etc.). Con este experimento se pretende observar en qué casos el sistema de telemedicina es viable y mejora el cuidado del paciente y en qué casos no, bien sea por el estado de salud del paciente, por su ubicación o por sus conocimientos, o bien por limitaciones en la coordinación de los profesionales sanitarios, limitaciones del sistema, etc.
Abstract
Care for complex chronic illnesses (diabetes, COPD, HIV/AIDS, etc.) increasingly requires more healthcare resources. In the case of HIV/AIDS, the problems faced by those affected have changed somewhat over recent years. Previously, the main efforts had been employed in increasing the time patients lived, whilst now the aim is to improve their quality of life, which is affected by a great range of factors (medical, psychological and social).
The Hospital VIHrtual is a telemedicine system that improves monitoring and care at home for HIV/AIDS patients. The main services offered by the system both for patients and healthcare personnel over the Internet are: consultations (via videoconferencing, chats and messages), viewing of patient data, managing appointments, remote pharmacy, virtual communities and a library. The main innovation in the system is that it comprehensively embraces the whole patient care process over the Internet (consultations, medical, psychological and social monitoring, medication, quality of life, coordination of the care team, etc.) without involving important technological innovations, instead innovating services, as they used tried and tested, low-cost technologies.
A pilot trial is being carried out at Barcelona's Hospital Clínic over two years with a total of 100 patients (50 in a trial group and 50 in a control group, crosschecked after a year) and 20 healthcare professionals (doctors, nurses, psychologists, psychiatrists, social workers, pharmacists, etc,), This experiment aims to highlight in which cases the telemedicine system is viable and improves patient care, and in which cases it does not, whether due to the state of the patient's health, their location or their knowledge, or due to the limitations in coordinating healthcare professionals, limitations in the system etc.
