Resumen
Fundamentos: Para facilitar la colaboración interprofesional y la Formación Continuada es necesario definir el perfil de competencias del Médico de Familia (MF) en Oftalmología y que este sea aceptado por los MF y los especialistas.
Métodos: Se envió a 20 Oftalmólogos del Area sanitaria de Pamplona un cuestionario con 42 competencias y 18 destrezas en el área de Oftalmología. Se solicitó su valoración sobre si cada una de ellas era parte del campo de actividad del MF. Las respuestas de 16 especialistas se comparan con las de 16 MF.
Resultados: 30 competencias y 12 destrezas son consideradas propias del MF por más del 75% de estos, frente a 22 competencias y 10 destrezas aceptadas por los oftalmólogos. Mas del 75% de estos expresan su desacuerdo en tres competencias: identificación de lesiones traumáticas de ambas cámaras, vigilancia de la tensión ocular en pacientes de riesgo, y del glaucoma crónica, y tratamiento inicial de las heridas penetrantes de globo ocular
y en tres destrezas: valoración del ángulo irridocomeal, dilatación de pupila y comprobación de la permeabilidad de vías
lagrimales. en 9 items se apreciaron diferencias estadísticamente significativas (P < 0.05) entre las respuestas de ambos grupos. Destaca entre ellas el reconocimiento y vigilancia de las Renitopatías en enfermedades vasculares y metabólicas.
Conclusiones: Nuestro estudio evidencia una dispandad de criterios que puede dificultar la colaboración interprofesional entre ambos grupos.Se recomienda estudiar los factores que pueden influir en el desacuerdo.
La lista elaborda consiituye un avance en la definición de perfil del MF en oftamología aunque sería necesario realiza más estudios.
Abstract
Background: In order to facilitate interprofessional collaboration
and continuing education, it is necessary to define the Family Doctor (PD) work profile in Ophthalmology and that this profile is accepted by FD and specialized doctors.
Methods: A questionnaire with 42 activities and 18 skills, belonging to the scope of Ophthalmology was sent to 20 ophtahnologists in the Health Area of Pamplona. They were asked to value each item and to express whether they thought it belonged to the FD activity scope. The answers from 16 specialized doctors are compared with the answers from 16 PD.
Results: 30 activities and 12 skills are considered to be a part of the PD scope by 75% of them, compared with 22 activities and 10 skiIls accepted by the ophtahnologists. More that 75% of these ones express their disagreement with three activities: identiíication of traumatic wounds in both cameras, surveiIlance of eye-pressure in patients at risk, surveillance of chronic glaucoma and early care of wounds in the eye gloh, and in three skills valuation of iridocomeal angle, enlargement of the eye pupil and veriíication of lacrimal conduct permeability. In 9 items, statistically significant differences were found (p 0.05) between both groups answers. Among them, the examinations and surveillance of retinopaties in vascular and metabolic diseases, stands out.
Conclusions: Our study shows a disparity of positions, which may raise difficulties to the interprofessional collaboration between both groups. It is recommended to study the factors, which might have an influente on this disagreement. The elaborated list constitutes a progress in the detition of FD profile in Ophtahnology, although
