Resumen
Fundamento: El patrón epidemiológico de la hepatitis A ha cambiado en los últimos años, y se observa una disminución de anticuerpos hnti-hepatitis A IgG (Anti-VHA) en las edades mas tempranas, lo que se acompañará en un futuro de un aumento de hepatitis sintomaticas. La prevención de la hepatitis A exige la aplicaci6n de normas de higiene ambiental: personal y la administración de vacunas o inmunoglobulina. Determinar la conveniencia de inmunizar activa o pasivamente con o sin detección previa de Anti-VHA, exigen determinar qué estrategia es la más eficiente.
Métodos: Se realiza un anlisis para determinar el umbral de prevalencia, donde la razón de eficiencia se establece
al comparar el coste unitario de inmunizar activa o pasivamente a la población con el coste de inmunizar
sólo a los Anti-VHA negativos por cribado previo, mediante la fórmula: Coste unitario de la inmunización activa
o pasiva (coste unitario del cribaje + coste de la inmunización activa o pasiva con inmunoglobulina inespecífica
en los Anti-VHA negativos). Los resultados se correlacionan con las prevalencias de Anti-VHA para grupos de edad encontrados en los estudios seroepidemiológicos publicados por Salleras (1992) y Pérez-Trallero (1994).
Resultados: El umbral de prevalencia, razón de eficiencias igual a 1, se sitúa en el 18% y 65% para la inmunización activa y pasiva respectivamente, lo que se corresponde con los grupos etarios entre 10-19 años y 20-29 años, en base a los datos seroepidemiológicos utilizados.
Conclusiones: Con prevalencias de Anti-VHA igual o superiores al 18% en la población la estrategia mas eficiente
es realizar determinación de Anti-VHA previa a la inmunización activa
Este umbral de prevalencia se desplaza
al 65% ante la inmunización pasiva. Por debajo de estas prevalencias es más eficiente inmunizar activa o pasivamente sin cribaje previo.
Abstract
Bockgmund: The epidemiological patmn of hepatitis A has changed in the last few years and B decrease of the
anti-hepatitis A antibodies IgG (Anti-HVA) have been observed at early ages, which will accompany in the future an
increase of symptomatic hepatitis. The prevention of hepatitis A requires a strict application of the norms of personal and environmental hygiene and the administration of vaccines or inmunoglobulins. In order to determine the convenience of inmunization actively or passively with or without the previous detection of Anti-HVA, requires the knowledge of with strategy is more efficient.
Methods: An analysis is carriout to determine the thershold of prevalence, where the reason of effciency is estabtished by compating the unit cost of inmunization either activeiy or passively of the population, with a cost of inmunizing only the negative Anti-HVA by previous screening, with the formula: the unit cost of the active or passive inmunization (unit cost of screening + cost of active or passive (inespecific inmunoglobuiine) inmunization in the negative Anti-HVA). The resuls correlate with the prevalence of Anti-HVA in age group founded in sero-epidemiological studies publied by Sallems (1992 and Pérez-Trallero (1994).
